Informed Consent to Telehealth Services

By using MDside, you acknowledge that you have read, understood, and agree to the following.
What Telehealth Is
Telehealth involves the use of secure electronic communications to enable licensed healthcare providers to evaluate, diagnose, and treat patients remotely — without an in-person visit. Your care through MDside is delivered primarily through asynchronous online intake ("store-and-forward"), and, where required, live communication with a provider.
Nature of the Services and Their Limits
- A licensed provider — never an automated system or AI — reviews your intake and decides whether treatment is clinically appropriate.
- A prescription is not guaranteed. The provider may decline to prescribe, request more information, or recommend in-person care.
- Telehealth has inherent limitations: the provider cannot physically examine you and relies on the accuracy and completeness of the information you provide. Inaccurate information can lead to an incorrect assessment.
- Telehealth is not appropriate for medical emergencies. If you are experiencing an emergency, call 911 or go to the nearest emergency department.
Your Responsibilities
- You confirm you are at least 18 years old and physically located in a state where MDside's providers are licensed.
- You agree to provide complete and accurate health information, including current medications, allergies, and relevant conditions.
- You agree to follow up with in-person care when advised and to seek emergency care when appropriate.
Benefits and Risks
Potential benefits include improved access to care, convenience, and reduced travel. Potential risks include, but are not limited to: technology failures or delays; that the provider may not have access to your complete medical records; and the limitations of remote evaluation described above. Privacy and security risks are mitigated but cannot be entirely eliminated when information is transmitted electronically.
Prescriptions, Pharmacies, and Compounded Medications
If a prescription is issued, it is sent to a licensed pharmacy for fulfillment. Certain medications offered through MDside may be compounded by a licensed compounding pharmacy. Compounded medications are not reviewed by the FDA for safety, effectiveness, or quality the way FDA-approved finished drugs are. Your provider will discuss whether a compounded option is appropriate for you.
Controlled Substances
Where MDside offers treatments that are controlled substances, they are prescribed only by providers holding the required DEA registration and only where permitted by applicable federal and state law, including telemedicine prescribing rules. Additional evaluation, lab work, or monitoring may be required.
Privacy
Your information is handled in accordance with our Privacy Policy and HIPAA Notice of Privacy Practices.
Consent and Withdrawal
You have the right to withhold or withdraw your consent to telehealth services at any time without affecting your right to future care or treatment. To withdraw consent, stop using the service and contact support@mdside.com.
By proceeding with an MDside intake, you certify that you have read this consent, that your questions have been answered to your satisfaction, and that you consent to receive care via telehealth.
